I hereby release and agree to hold Hearts, Hands and Hope harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of Hearts, Hands and Hope, or that may otherwise arise in any way in connection with Hearts, Hands and Hope. I understand that this release discharges Hearts, Hands and Hope from any liability or claim that I, my heirs, or any personal representatives may have against Hearts, Hands and Hope with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, Hearts, Hands and Hope. I understand that closed-toed shoes are required to be worn at all times in the warehouse. I hereby release Hearts, Hands, and Hope from any liability for injury resulting from my failure to comply with this requirement. This liability waiver and release extends to all owners, board members, partners, and employees.

I hereby grant Hearts, Hands and Hope and its representatives the irrevocable right and permission to photograph, videotape, and record my likeness and voice, and to use, reproduce, publish, and distribute these images and recordings in any media format (including print, digital, online, social media, and promotional materials) for any lawful purpose without compensation, approval, or additional consent from me, and I waive any right to inspect or approve the final use of such images or recordings.
